Take your nursing expertise into the community by providing essential care for patients in their homes. This full-time, 13-week contract position is an excellent opportunity for Registered Nurses with home health backgrounds who thrive in field-based settings and value independence, flexibility, and patient-centered care.

Schedule & Hours:

  • Monday to Friday, 8:00 AM – 5:00 PM
  • Caseload: Expect around 30 points per week, typically 3–5 patient visits per day

    • Desired Qualifications:

  • Active RN license (in good standing) for the state of practice
  • At least 1 year of recent home health experience, ideally in the field
  • Strong proficiency with Home Care Home Base (HCHB) electronic documentation
  • OASIS assessment experience is required
  • Reliable transportation, valid driver’s license, and proof of auto insurance

    • Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop personalized care plans
  • Deliver skilled home health nursing, including wound care, medication administration, IV therapy, and chronic disease management
  • Monitor and respond to changes in patient condition, communicating effectively with the care team and physicians
  • Provide education to patients and families on disease management, medications, and preventive strategies
  • Ensure accurate and timely charting in HCHB
  • Collaborate closely with therapists, social workers, and home health aides for coordinated patient care
  • Participate in interdisciplinary meetings, case conferences, and ongoing training
  • Maintain strict adherence to regulatory and agency documentation and care standards

    $2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
