Schedule/Hours:

    • Qualifications:

  • Current and valid RN license
  • Minimum of 2 years of recent case management experience as an RN
  • Strong knowledge of utilization review, discharge planning, and care transitions
  • Exceptional communication, organizational, and critical thinking abilities
  • Ability to collaborate effectively with interdisciplinary teams
  • BLS certification required

    • Responsibilities:

  • Assess, plan, and coordinate care needs for patients
  • Facilitate safe and effective discharge planning and transitions of care
  • Partner with physicians, social workers, and other healthcare professionals to develop individualized care plans
  • Conduct utilization reviews and ensure compliance with regulatory requirements

  • Maintain thorough and timely documentation of all case management activities
