Schedule/Hours:

  • 5x8-hour shifts (40 hours per week guaranteed)
  • Rotating 36/48 schedule: 7a-7p every Wednesday & Thursday, every other Friday, Saturday, Sunday
  • No weekend requirements
  • Holidays rotated as needed

    Qualifications:

  • Active LMSW or LCSW + current MS State License
  • Minimum 2 years acute inpatient medical case management (including at least 1 year in the last 3 at a large, 500+ bed, Level 1/Academic Medical Center)
  • Prior travel experience in a Level 1 facility within the last 3 years
  • Recent acute care & ED discharge planning experience (within the last 2 years)
  • Proficiency in EPIC charting and strong computer skills
  • BLS certification
  • Compliance with flu vaccine and fingerprint requirements (documentation required for medical exemption)
  • Primary Responsibilities:

  • Acute inpatient case management from admission to discharge
  • Coordinate with interdisciplinary teams for safe and effective discharge planning
  • Manage a caseload of 15-20 patients daily
  • Collaborate with respiratory therapy, pharmacy, and other departments as needed
  • Serve as a resource for patients with complex placement needs
