Schedule/Hours

  • 5x8 hour shifts (40 hours/week), 36/48 rotating schedule: 7a-7p every Wednesday & Thursday plus every other Friday, Saturday, and Sunday
  • Guaranteed 40 hours per week
  • Rotating holiday coverage as needed
  • No weekend requirements
  • Qualifications

  • Active LMSW or LCSW and current MS State License
  • Minimum 2 years recent experience in acute inpatient medical case management (at least 1 year in a 500+ bed, level 1/academic facility)
  • Prior travel experience in a level 1 facility within the past 3 years
  • Strong proficiency in EPIC and computer skills
  • Inpatient acute case management and discharge planning required
  • BLS Certification and flu vaccine (medical exemption requires documentation)
  • Able to efficiently integrate with minimal training
  • Responsibilities

  • Manage a caseload of 15-20 patients/consults per day in an acute, high acuity environment
  • Coordinate discharge plans and work closely with interdisciplinary teams, including RT and Pharmacy
  • Execute complex placements and navigate financial assistance criteria
  • Maintain accurate and timely documentation in EPIC
  • Adhere to professional dress code (any color scrubs or business attire)
