Schedule/Hours:

  • Flexible weekday schedule, self-managed appointments
  • Weekend and call shifts available but not required
  • Holiday work: 75% of holidays expected
  • Key Qualifications:

  • Active RN license and BLS certification
  • Valid driver’s license and proof of insurance
  • Minimum one year of home health experience
  • Proficiency with Homecare Homebase (required)
  • Skilled in wound care, IV therapy, pleurx, wound vacs, assessment and teaching chronic disease management
  • Capable of managing up to 30 episodes concurrently
  • Strong autonomy and organizational skills
  • Role Responsibilities:

  • Complete a minimum of 6 patient visits daily across a defined service region
  • Deliver direct patient care, education, and skilled interventions in varied home environments
  • Manage OASIS documentation, case management, and care coordination
  • Collaborate with clinical supervisors and participate in care plan development
  • Schedule patient visits and maintain timely, thorough documentation
  • Utilize provided Samsung Note 9 and EPIC Rover for records and communication
