Deliver high-quality, compassionate care directly to patients in their own homes as a Registered Nurse specializing in Home Health. This contract opportunity offers a dynamic and rewarding environment for nurses who excel in independent, field-based work and are committed to maintaining exceptional clinical standards.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Responsibilities:

  • Complete comprehensive OASIS assessments and design individualized patient care plans.
  • Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient conditions, communicate changes promptly to healthcare team members, and adjust care plans as needed.
  • Educate patients and their families about disease management, medications, and preventive care.
  • Maintain timely, accurate documentation using Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, social workers, and home health aides.
  • Participate in staff meetings, case conferences, and ongoing education.
  • Adhere to agency policies, regulatory standards, and best practices for home health care.

    • Qualifications:

  • Active RN license (in good standing) in the state of practice.
  • At least 1 year of recent home health field experience.
  • Proficiency with Home Care Home Base (HCHB) and OASIS required.
  • Strong assessment skills and ability to manage a caseload of 25–32 points per week.
  • Excellent communication, organizational, and time management abilities.
  • Reliable vehicle, current driver’s license, and auto insurance.
  • BSN preferred; understanding of Medicare/home health compliance a plus.

    $2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
