An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health. In this field-based position, you will deliver patient-centered nursing care directly to individuals in their homes, fostering positive health outcomes while working with a multidisciplinary healthcare team.

Schedule:

  • Monday–Friday, 8:00 AM–5:00 PM
  • 13-week contract duration

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans for each patient.
  • Deliver skilled nursing care: wound management, medication administration, IV therapy, and chronic disease oversight.
  • Assess and monitor patient conditions, communicate significant changes promptly to physicians and the care team.
  • Educate patients and families about diagnoses, medications, treatments, and preventative care measures.
  • Document all care activities and patient interactions accurately using the Home Care Home Base (HCHB) system.
  • Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for seamless care coordination.
  • Participate in interdisciplinary meetings and pursue continuing education as required.
  • Uphold compliance with all regulatory and agency standards in patient care delivery.

    • Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum one year of recent Home Health field experience.
  • Proficient in Home Care Home Base (HCHB) and OASIS assessment.
  • Reliable transportation, valid driver’s license, and auto insurance are required.
  • Excellent communication, clinical assessment, and organizational skills.
  • Ability to work independently and manage a dynamic field-based schedule.
  • BSN and experience managing 25–32 points per week are preferred.
  • Knowledge of Medicare and home health compliance standards is advantageous.

    $2,134 - $2,258/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
