A rewarding opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health. This contract position focuses on providing skilled nursing care within patient homes, supporting quality and patient-centered outcomes. The role is well-suited to a motivated professional who thrives independently while maintaining clinical excellence.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize changes in condition, and communicate effectively with medical staff.
  • Educate patients and families about disease processes, medications, treatments, and preventative strategies.
  • Accurately document care using Home Care Home Base (HCHB).
  • Coordinate with physical therapists, occupational therapists, social workers, and aides to deliver comprehensive care.
  • Participate in case conferences, staff meetings, and ongoing professional development.
  • Adhere to all regulatory and best-practice standards in both patient care and documentation.

    • Qualifications:

  • Active and unrestricted RN license in the state of practice.
  • At least one year of recent Home Health experience; field experience preferred.
  • Proficient in Home Care Home Base (HCHB) and OASIS documentation.
  • Effective clinical assessment and problem-solving abilities.
  • Reliable transportation, valid driver’s license, and current auto insurance.
  • Strong time management, organizational, and communication skills.
  • BSN preferred.
  • Experience managing a caseload of 25–32 points per week and knowledge of Medicare/home health compliance standards are highly desired.
