Deliver high-impact care right where it matters most—at home. A skilled Home Health Registered Nurse (RN) is needed to provide patient-centered, evidence-based nursing services directly in patients’ residences. This contract opportunity calls for compassionate and self-driven professionals comfortable working autonomously in the field and maintaining a high standard of clinical care.

Schedule & Details:

  • Full-time, 13-week contract
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Caseload: 25–32 points per week (typically 3–5 patient visits per day)
  • Field-based, with daily travel to patient homes (mileage reimbursement provided)
  • Electronic documentation via Home Care Home Base (HCHB)
  • OASIS assessment tool utilized

    • Role Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized plans of care.
  • Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, recognize and report changes to the interdisciplinary care team and physicians.
  • Educate patients and families on medications, disease management, and preventive health strategies.
  • Maintain meticulous and timely patient records using HCHB.
  • Work collaboratively with therapists, social workers, and home health aides to promote coordinated, holistic care.
  • Participate in case conferences, meetings, and ongoing training as required.
  • Adhere to all regulatory and professional standards.

    • Preferred Qualifications:

  • Active RN license (in good standing) in Kentucky
  • Minimum 1 year recent home health nursing experience (field experience strongly preferred)
  • Hands-on proficiency with HCHB and OASIS
  • BSN preferred
  • Prior experience managing a caseload of 25–32 points per week
  • Reliable vehicle, valid driver’s license, and auto insurance
  • Strong assessment, communication, and time management skills
  • Familiarity with Medicare and home health compliance a plus
