Deliver compassionate, patient-centered care as a Home Health Registered Nurse, dedicated to supporting individuals in the comfort of their own homes. This contract position is ideal for motivated RNs who excel in autonomous fieldwork and enjoy building trusted relationships with patients and their families. The role emphasizes comprehensive in-home assessments, collaboration with interdisciplinary care teams, and ongoing patient education to foster optimal health outcomes.

Schedule & Job Details:

  • Full-time contract, 13 weeks
  • Monday – Friday, 8:00 AM – 5:00 PM
  • Caseload: 25–32 points per week (typically 3–5 patient visits each day)
  • Setting: Field-based (travel to patient homes; mileage reimbursement provided)
  • Electronic documentation with Home Care Home Base (HCHB); OASIS assessment tool

    • Key Accountabilities:

  • Complete thorough OASIS assessments and tailor individualized care plans
  • Deliver skilled interventions: wound care, IV therapy, medication administration, and chronic disease management
  • Monitor patient status, promptly report changes, and document progress using HCHB
  • Educate patients and families on diagnosis, medication adherence, and preventive strategies
  • Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides
  • Participate in team meetings, case conferences, and ongoing professional development
  • Maintain compliance with regulatory requirements and standards

    • Desired Qualifications:

  • Active, unencumbered RN license (state of practice)
  • At least 1 year of recent Home Health RN experience (field experience strongly preferred)
  • Proficient with Home Care Home Base (HCHB) and OASIS
  • Strong assessment, critical thinking, and independent time management skills
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Excellent communication and organizational abilities
  • BSN and/or experience managing 25–32 points weekly preferred
  • Knowledge of Medicare and home health compliance is a plus
