Deliver essential patient care directly to individuals in their homes as a Home Health Registered Nurse (RN). This contract opportunity is ideal for compassionate nurses who thrive in autonomous, field-based roles and are dedicated to providing high-quality, patient-centered care.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Role Overview: In this field-based position, you will conduct comprehensive OASIS assessments, oversee individualized care plans, and ensure patient progress through regular evaluations. You’ll collaborate with multidisciplinary teams, educate patients and families, and deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management.

    Core Responsibilities:

  • Complete initial and ongoing OASIS assessments with accurate documentation in Home Care Home Base (HCHB)
  • Develop, implement, and modify patient-specific care plans
  • Provide hands-on clinical care (wound care, IVs, medication management)
  • Monitor patient progress, communicate changes to healthcare providers, and update care plans accordingly
  • Educate patients and families on health conditions, treatments, and preventive care measures
  • Collaborate with therapists, social workers, and home health aides for coordinated care delivery
  • Maintain compliance with agency and regulatory standards for home health services
  • Participate in team conferences and continuing education as required
  • Daily travel to patient homes within your assigned territory; mileage reimbursement is provided
  • Required Qualifications:

  • Active and unrestricted RN license in the state of practice
  • At least 1 year of recent Home Health nursing experience (field-based strongly preferred)
  • Proficiency with HCHB and OASIS documentation
  • Strong assessment, organizational, and communication skills
  • Reliable transportation, valid driver’s license, and auto insurance
  • Preferred Qualifications:

  • BSN degree
  • Experience with a caseload of 25–32 points per week
  • Working knowledge of Medicare and home health regulations
