Experienced and compassionate Registered Nurses with a background in home health are needed to deliver high-quality patient care in a field-based setting. This position is responsible for providing comprehensive nursing assessments, managing individualized care plans, and collaborating with interdisciplinary teams to support patient health and recovery at home. The ideal candidate values autonomy, demonstrates superior organizational skills, and excels at clinical communication and patient education.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Contract Length:

  • 13 weeks, full-time
  • Caseload:

  • 25–32 points per week (approximately 3–5 patient visits per day)
  • Position Details:

  • Field-based work supporting patients in their homes within an assigned territory
  • Daily travel required (mileage reimbursement provided)
  • Documentation conducted via Home Care Home Base (HCHB)
  • OASIS assessment tool utilized
  • Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop patient-centered care plans
  • Provide skilled nursing services such as wound care, IV therapy, chronic disease management, and medication administration
  • Monitor patient status, communicate changes to physicians and care team members
  • Educate patients and families on treatment, medications, and preventive care
  • Maintain timely and accurate clinical documentation within HCHB
  • Collaborate with therapy, social work, and aide teams to ensure coordinated care
  • Attend staff meetings, case conferences, and ongoing education as needed
  • Meet all regulatory and compliance standards
  • Qualifications:

  • Current RN license (active and in good standing)
  • At least 1 year of recent home health nursing experience (field experience preferred)
  • Experience with Home Care Home Base (HCHB) and OASIS
  • Solid clinical assessment and critical thinking abilities
  • Dependable transportation, valid driver’s license, and auto insurance
  • BSN preferred
  • Experience managing Medicare compliance and caseloads of 25–32 points per week is desirable
