Experienced and dedicated Registered Nurses are invited to deliver high-quality care to patients in their homes. This field-based role offers the opportunity to make a meaningful impact through skilled nursing while working independently within a supportive clinical framework.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM

    • Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop patient-centered care plans.
  • Provide professional nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor and document patient progress using Home Care Home Base (HCHB); ensure timely and accurate entries.
  • Communicate patient status updates and significant changes to the broader care team and physicians.
  • Educate patients and families about medications, disease processes, treatment protocols, and preventive care strategies.
  • Collaborate with an interdisciplinary team including therapists, social workers, and aides for coordinated care.
  • Participate in case conferences, clinical meetings, and ongoing training activities.
  • Maintain compliance with regulatory standards and best practices in home health.

    • Qualifications:

  • Current, active RN license in good standing in the state of practice.
  • At least 1 year of recent home health nursing experience; field experience is preferred.
  • Fluency with Home Care Home Base (HCHB) and OASIS assessment tools.
  • Strong clinical assessment skills, critical thinking, and effective problem-solving abilities.
  • Excellent communication, time management, and organizational expertise.
  • Reliable transportation, active driver’s license, and current auto insurance.
  • BSN and prior caseload management (25–32 points per week) are preferred.
  • Understanding of Medicare/home health compliance standards is a plus.
