A skilled and dedicated Registered Nurse is needed for a contract home health assignment, providing patient-centered care in a field-based role. This opportunity allows you to make a significant impact by delivering comprehensive nursing services in patient homes and maintaining a high standard of clinical care.

Schedule:

  • Monday through Friday, 8:00 AM – 5:00 PM
  • Full-time, 13-week contract
  • Caseload: 25–32 points per week (approximately 3–5 patient visits per day)

    • Responsibilities:

  • Perform OASIS assessments and develop tailored care plans for each patient
  • Deliver skilled nursing care such as wound care, IV therapy, medication administration, and disease management
  • Monitor and document patient status, promptly reporting changes to the interdisciplinary care team and physicians
  • Educate patients and families on treatment plans, medications, and preventive strategies
  • Maintain accurate, timely electronic records using Home Care Home Base (HCHB)
  • Collaborate with other clinicians to ensure cohesive care and attend relevant meetings and case conferences
  • Adhere to regulatory and best-practice standards for documentation and care delivery

    • Qualifications:

  • Active RN license in the state (in good standing)
  • Minimum of 1 year of recent home health experience (field-based roles preferred)
  • Experience with Home Care Home Base (HCHB) and OASIS assessments is required
  • Strong clinical and critical thinking abilities, excellent communication, and organizational skills
  • Reliable transportation, current driver’s license, and auto insurance required
  • BSN and experience managing 25–32 points per week preferred
  • Knowledge of Medicare and home health compliance is a plus
