Bring your commitment and expertise in hospice nursing to a meaningful role supporting patients and families through end-of-life care. This contract position involves providing comprehensive field-based hospice services, focusing on comfort, dignity, and compassionate support within home and residential settings.

Schedule:

  • Monday – Friday, 8:00 AM – 5:00 PM
  • Occasional weekend or on-call rotation as required
  • Key Responsibilities:

  • Complete thorough nursing assessments and develop individualized hospice care plans
  • Administer direct skilled nursing care with an emphasis on symptom management and pain control
  • Deliver medications, treatments, and interventions in line with physician directives and hospice standards
  • Monitor and promptly report changes in patient condition to clinical leaders and team members
  • Educate patients, families, and caregivers about disease progression, comfort measures, and emotional support options
  • Partner with interdisciplinary team members (social workers, aides, chaplains, volunteers) to ensure holistic, patient-centered care
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB)
  • Participate in team meetings, case conferences, and quality improvement initiatives
  • Ensure compliance with hospice policies and regulatory requirements
  • Qualifications:

  • Active RN license in good standing
  • Minimum 1 year of recent hospice care experience
  • Proficient with Home Care Home Base (HCHB) documentation
  • Strong clinical judgment and interpersonal communication skills
  • Thorough knowledge of end-of-life care best practices
  • Reliable vehicle, valid driver’s license, and proof of auto insurance
  • Demonstrated independence, organizational skill, and compassion
  • Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Experience in palliative or end-of-life care
  • Awareness of Medicare hospice regulations
