Step into a dynamic opportunity as a Nurse Practitioner providing home health assessments across Island County, WA, including areas such as Oak Harbor, Camano, and Whidbey Island. In this per diem position, you’ll focus on comprehensive in-home health risk assessments for Medicare Advantage members, supporting their post-discharge care and annual evaluations.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment
  • Key Responsibilities:

  • Conduct detailed home health risk assessments for Medicare Advantage plan participants
  • Evaluate patients’ prior and current medical history, existing medical conditions, and medication lists
  • Educate patients on their medical conditions and medications during each visit
  • Identify gaps in care and provide targeted recommendations to improve outcomes
  • No need to create treatment plans or prescribe medications, allowing you to focus exclusively on assessment and education
  • Qualifications:

  • Active Nurse Practitioner license in Washington
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of six months current NP experience preferred
  • MSN degree from an accredited school of nursing
  • The average home health risk assessment takes approximately 45 minutes. You will be scheduled for cases within Oak Harbor and surrounding Island County locations, offering a variety of patient encounters within each shift.

    This role offers generous evaluation-based compensation, including $80-$90 per assessment completed, a mileage stipend, and additional administrative time support.
