Step into a dynamic per diem opportunity delivering home health risk assessments across Cowlitz County. As a Nurse Practitioner, you will play a key role in supporting Medicare Advantage plan members through comprehensive annual health evaluations or post-discharge assessments. Each assessment typically requires about 45 minutes and provides valuable contributions to the continuum of patient care.

  • Minimum weekly or bi-weekly hourly commitment required
  • Key Responsibilities:

  • Perform in-home annual risk assessments for Medicare Advantage plan beneficiaries
  • Assess prior and current medical history, existing medical conditions, and medication regimens
  • Provide patient education regarding health conditions and medications
  • Identify gaps in care and recommend appropriate interventions for follow-up
  • Note: This role excludes developing treatment plans or prescribing medications
  • Qualifications:

  • Current Nurse Practitioner license in Washington
  • Board Certification from AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of 6 months’ recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program
  • This position offers attractive pay, with evaluations compensated at $80–$90 each. Additional mileage and administrative time stipends are provided for your convenience and support.
